Emergency Department Patient Navigator Full Time Days Position Summary

Facilitates and assists qualifying patients and families through the healthcare continuum with a focus on Emergency Department navigation services. The role will be focused on providing the following services:

  • Assistance with Access to Primary Care
  • Addressing Systemic Barriers to Accessing Care and Managing Chronic Disease
  • Connecting with Available Local Resources
  • Providing Administrative Services including management of an ED Navigation Registry

THE EMERGENCY DEPARTMENT PATIENT NAVIGATOR FULL TIME DAYS CANDIDATE WILL POSSESS THE FOLLOWING EDUCATION, LICENSE/CERTIFICATIONS, AND EXPERIENCE.

The role will be a remote position for multiple sites of care across South Carolina and will require excellent communication skills and knowledge of resources available within various communities. It will also require an ability to understand the healthcare delivery system and adapt to various patient populations. This position will also help to coordinate referrals; provide a significant level of patient education; and guide patient care pathways. Strong organizational skills are a must along with a passion to work with people.

Qualifications:

  • Critical thinking and problem-solving skills
  • Assessment skills to include the assessment of psychosocial needs
  • The ability to plan and organize time
  • The ability to effectively utilize resources to meet patient needs.
  • Relationship management and strong professional communication skills

Education/Other Qualifications:

  • Bachelors Degree in Social Work or Nursing
  • 1+ years in a clinical or social services setting working directly with patients
  • Must have computer skills and dexterity required for data entry and retrieval of information
  • Effective verbal and written communication skills and the ability to present information clearly and professionally
  • Must be proficient with Windows-style applications, various software packages specific to role and keyboard
  • Strong interpersonal skills
  • Strong oral and written communication skills
  • Proven leadership skills, ability to think critically, problem-solve, work independently, self-initiate, multi-task, and work effectively in a team environment
  • Highly organized and detail-oriented
