GENERAL SUMMARY: Under the general supervision of a Registered Nurse and according to established standards, policies, and procedures, provides direct nursing care to assigned patients. Actual duties and responsibilities may vary depending on assigned work area and patients. Ensures quality and safe delivery of care to patients in the following age groups: All ages; Neonate; Infancy; Toddler; Preschool; School age; Adolescent; Adult; Geriatric.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Direct Patient Care Using the Nursing Process
A. Assessment
1. Collects data and identifies specific physical, emotional, and cultural needs of patients.
2. Interviews patient to obtain a health history and other basic information.
3. Identifies overall needs of patients.
4. Observes the patient and communicates significant findings to the nursing team.
5. Identifies appropriate resources which patients may need.
6. Identifies basic pathophysiology of assigned patients.
B. Planning
1. With assistance, collaborates in and contributes to the development/updating of care planning and teaching plans for patients and/or families.
2. Establishes priorities and organizes an assignment.
C. Implementation
1. Safely performs basic therapeutic and preventive nursing procedures, incorporating fundamental biological and psychological principles in giving individualized care.
2. Shows respect for the dignity of individuals.
3. Applies basic communication techniques.
4. Demonstrates the ability to do incidental teaching during routine care and to support and reinforce the teaching plan for a specific patient.
5. Uses lines of authority and communication within the work setting.
D. Evaluation
1. Makes necessary adjustments in care and seeks guidance as needed.
2. Identifies own strengths and weaknesses and seeks assistance for improvement of performance.

Documentation
A. Documents/communicates information regarding the patient’s status, problems, and complaints to R.N.
B. Documents evaluation of outcomes of nursing intervention.
C. Maintains accurate patient, unit, and Hospital records.

Communications
A. Communicates information regarding patient’s status, problems, and/or complaints to appropriate healthcare team member.
Environment
A. Works to ensure a clean and safe environment at all times for patients and other staff. Applies restraints and/or uses side rails appropriately. Uses aseptic techniques or universal precautions as warranted or prudent, and disposes of hazardous waste in the proper manner.
B. Maintains current knowledge of safety issues, including infection control, fire, electrical safety, and body mechanics to reduce or eliminate the risk of personal illness or injury. Reports hazardous conditions or risks promptly.
Education
A. Maintains an up-to-date knowledge of appropriate nursing skills, technology, and patient care issues pertinent to the unit to which the LPN is assigned. Demonstrates a sound knowledge base and acts according to established standards of practice.
B. Provides basic information to patients/family regarding the aspects of care and/or Hospital procedures. Works to ensure optimal patient knowledge and encourages patient cooperation and participation in the recovery process.
C. Participates in in-service education programs as appropriate/required.
D. Seeks out and takes advantage of learning situations and opportunities for own continuing education.

OTHER JOB KNOWLEDGE, EXPERIENCE AND REQUIREMENTS:
1. The level of knowledge equivalent to that ordinarily acquired through completion of an accredited LPN program.
2. Current licensure as an LPN in Massachusetts.
3. Three to six months of experience as an LPN.
